Terms of Service


Private Practice Check-in ("PPCI") is a subscription service offered by SLS Therapy, Inc. ("SLS") that includes the use of a tablet, stand,
client and server software, allowing clients to check-in to practitioner offices.

SUBSCRIPTION

Monthly Subscription
By purchasing a monthly subscription, you agree to an initial and recurring monthly subscription fee at the then-current monthly subscription rate,
and you accept responsibility for all recurring charges until you cancel your subscription.  You may cancel your subscription at any time,
subject to the terms of our cancellation policy.

Once you subscribe, SLS will automatically process your monthly subscription fee in the next billing cycle.  SLS will continue to automatically
process your montly subscription fee each month at the then-current monthly subscription rate, until you cancel your subscription.  You can cancel
your subscription at any time from the payments section of the "administration page", from the "Log in" link on https://www.ppcheckin.com.

Annual Subscription
By purchasing an annual subscription, you agree to an initial pre-payment for one full year of service.  After one year and annually therafter,
you will be billed a recurring annual subscription renewal fee at the then-current annual subscription rate.  You may cancel your annual
subscription at any time, subject to the terms of our cancellation policy.

We will bill your credit card for the annual subscription renewal fee in the first month of your next annual subscription.

Missed Payment
If your credit card is rejected, you will be notified (via e-mail).  Billing information can be updated on the website.  If we
are unable to collect payment within five (5) days of notification, we will disable the service.

In all cases, active subscribers will be notified of any price changes (via e-mail) prior to the billing cycle in which the fee will change.

SUPPORT

SLS will keep PPCI online.  If there is scheduled downtime, it will be between 12:00am PT and 4:00am PT (3:00am ET and 7:00am ET).
For unscheduled downtime (malfunctions), SLS will endeavor to repair any problems as quickly as possible.  If the service is unavailable
outside of scheduled downtime for more than 1/2 hour, a credit for an additional day of service will be granted upon request from you.

If our equipment at your facility malfunctions, we will send you new equipment.  At that time, we will automatically credit you
one extra month of service.

PPCI requires WiFi Internet in order to function.  THE SERVICE WILL NOT WORK AT ALL IF THE INTERNET AT THE PRACTICE IS DOWN, OR IF
THE TABLET CANNOT CONNECT TO THE INTERNET.  SLS CANNOT BE RESPONSIBLE FOR DOWNTIME DUE TO INTERNET OUTAGES OR NON-SLS EQUIPMENT
MALFUNCTIONS AT THE CLIENT SITE.

The tablet is meant to be plugged-in to a power outlet while in use.  It is not meant to be running on battery power during normal
use in the office.  SLS cannot be responsible for downtime due to allowing the battery to fully drain.

TERMINATION OF SUBSCRIPTION

You may cancel your service at any time from the website.  When you do this, from that point forward there will be no more charges to
your credit card.  All previous payments are non-refundable.  Your service will continue to operate until your next payment would be due.

SLS may cancel your service with two week's notice, as of the next payment due date after that two weeks.  On that payment due date, the
service will stop working.

SLS may also cancel your service for non-payment, as described above in the section titled "Missed Payment".

EQUIPMENT

No equipment will be sent to you until you give us your credit card information.  Signing up for an account does not require
a credit card, and will allow you to set up your options BEFORE the equipment is shipped.

Filling out of your payment information does not begin your subscription, and you will not be charged subscription fees.  However, it
will cause our equipment to be shipped to your address.  You will be expected to activate a subscription soon after you receive the
equipment.  If you do not activate a subscription within two (2) weeks of receiving the equipment, you need to return the equipment
immediately.  IF YOU DO NOT CREATE A SUBSCRIPTION AND SLS DOES NOT RECEIVE THE EQUIPMENT BACK WITHIN THIRTY (30) DAYS, YOUR CREDIT CARD
MAY BE CHARGED THE RETAIL PRICE OF THE EQUIPMENT.  SLS will work with you to facilitate the return.  We want the equipment back.


RETURN OF EQUIPMENT

When your service is canceled or your subscription ends, you will receive shipping supplies from SLS.  This will include postage, and may
include shipping boxes, depending on what you already have from receiving the equipment.  You agree to return the equipment owned by SLS within
thirty (30) days of receiving the shipping supplies.  FAILURE TO RETURN THE EQUIPMENT WITHIN THIS TIME MAY RESULT IN SLS CHARGING
YOUR CREDIT CARD FOR THE RETAIL PRICE OF THE EQUIPMENT.  SLS will work with you to facilitate the return.  We want the equipment back.
